1. filled and signed Application form

2. Passport: issued within the last 10 years, copied all previous (valid and expired) visas and used pages.

3. One passport size photography: white background, frontal view (3,5 x 4,5 cm)

4. Travel medical insurance

5. Flight reservation

6. Reservation of hotel confirmed and valid

7. Original employment letter, not less than 1 month old, including position, salary and form of payment, duration of leave and employers ‘guarantee of resuming functions after return to Lebanon/ Proof of registered business + commercial circular.

8. Students: Original enrollment letter less than 1 month old + a copy of University ID.

9. Original stamped up-to-date personal bank account statement covering transactions for the last 3 months only. Lebanese bank account, ONLY “Fresh dollar” account accepted.

10. Family Register: Original translated to English or Czech, not older than 6 months.
